Individualized plans and assessments.
Before your loved one moves in, we fully assess their needs: everything from overall physical and mental health, to capacity for self-care and social participation. From there, we determine a temporary Individual Service Plan (ISP) for once they’re admitted.
Within three days of arriving, each resident is evaluated for his or her ability and limitations for facility emergency evacuation. Within 30 days of living here, we’ll develop a complete ISP that’s reviewed annually, or as soon as there is a significant change in their condition.
